@Rajiv, these do not look like Planaria..try and post a better picture.
If they are indeed planaria , there is NO way to kill them totally , you can however keep them under check.
1. Reduce feeding and clean the tank properly every week. Should immediately reduce their numbers.
2. Put small fish - guppies , tetras - they will eat them like crazy.
3. Use Fenbendazole if the epidemic is an acute one. Mix Fbz with warm water, strain the contents (as Fbz dissolves poorly in water) and add to the tank. If the tank has invertebrates , do not use as the entire population will be wiped out. Where do you get Fbz ? Go to a pet shop which has stuff for dogs. Ask for dog deworming medicine.
