Seems like Planaria. Keep on doing frequent water changes.

They are not planaria. I changes 50% water twice a week.

They are nematodes and they are harmless. Their life cycle is very short and they should disappear within a few days. Shrimps and Plecos like to eat them.
In nature they are the lowest in the food chain, surviving on dead stuff and bio-debris thereby keeping water bodies clean, so...don't be angry at them ;-)
